Both sketches are correct , they are from BobFarlaine's book . The revolve axe has to be a bit further then.

The side plane on your sketch should look like the attached, then use the "Revolve" tool to create the first model section. From the model top section create your second sketch plane to extrude down the offset boss with slot, two threaded holes, and counterbore hole..
Bob, mention about irregularities between the drawing and the model, which is aggregable. You can choose bring up the topic with your instructor, or choose one of the shapes to revolve.
