How to add custom tool bar in solidworks 2011?
Not sure if you can add a custom toolbar.. but you can add commands to and existing toolbar.

Step 1:
Right click in the open grey space near the top & click customize

Step 2:
Find the commands you would like to add and click & drag them to an existing toolbar

Step 3:
Repeat Step 2 until you add all the commands you need
